Materials needed for I-20 or Change of Status:
- Official Copies of School Transcripts in a sealed envelope sent by the school (**English Translation)
These must clearly state each credential, provide U.S equivalent, and provide the U.S. credit and grade equivalent for each subject. (Personal copies of school transcripts will not be accepted)
- Copy of Passport and / or Birth Certificate
- Bank Statement / Affidavit Support of person responsible for paying tuition and fees.
- Name, Address, and Phone Numbers of Guardian in Houston
- Notarized letter from the guardian stating responsibility for student
- Complete Foreign Address
- At least (2) written character references in English translation
- Copy of all current medical/immunization records in English translation
**Please contact the school office if you have questions regarding the English translation of transcripts. We have a list of available certified agencies. No students will be issued I-20's without complete transcripts being received by Memorial Hall.
***There will be a $500.00 non-refundable fee for each I-20 form processed by Memorial Hall.
****Sevis I-901 REQUIRED FEE - $100.00
All students receiving their F-1 Visa's will be required to complete no less than two complete concurrent semesters with Memorial Hall School. Annual Tuition and fees for these students must be paid in full and are non-refundable.
